We currently accept new patients by phone. Our team will walk you through everything.
Reach us at (281) 333‑9200 Monday–Friday, 8:30 AM – 5:00 PM. Our staff will help find an appointment time that works for you at our Houston or Friendswood location.
Save time at your visit by completing your new patient forms in advance. Download them below, or pick them up at the office on the day of your appointment.
Bring your insurance card, a valid photo ID, a list of current medications, and any relevant prior medical records or test results. See the full checklist below.
Arrive 15 minutes early for your first visit. Our team will make sure you're comfortable and that your care plan gets off to the right start.

Day of Your Visit
Being prepared helps us make the most of your appointment time.
A valid government-issued ID such as a driver's license or passport.
Bring your current insurance card. We'll make a copy at check-in.
All current prescriptions, OTC medications, supplements, and dosages.
Prior cardiology reports, EKGs, echocardiograms, or lab results if available.
Your downloaded and filled-out intake forms, or arrive 15 min early to complete them.
If your insurance requires a referral from your primary care physician, bring it with you.
